Vulnerable Person Database Registration Program

In order to better serve our vulnerable residents, Goodyear Police Department is implementing a Vulnerable Population Database Registration Program.  This voluntary registration will provide critical information for responding officers, including a photo of the resident and recommendations on how to approach, if applicable.

Telecommunications will use the information to produce computer alerts for names and addresses associated with individuals with autism/Asperger’s syndrome, deaf/low hearing, Alzheimer’s disease/dementia, diabetes, intellectual disability, and other diagnosed mental health issues identified by the resident.

How do I submit a registration form?

Any city of Goodyear resident interested in registering an individual from their household into the vulnerable population database should:

  • Fill out the pdf registration form, print and mail form and current photograph to: Goodyear Police Department Vulnerable Population Database Program, Attention Telecommunications. 11 N. 145th Ave. Goodyear, AZ 85338.
  • Or, drop off completed form and current photograph at the Police Department Operations Building front desk in an envelope clearly labeled "Vulnerable Population Database Program," Attention Telecommunications. Lobby hours: M-F 8am-5pm.
  • Or, send an email with completed form and current photograph of the individual attached. 

Important: Please include a recent photo of the individual with your registration form.
